Asymptomatic precursor — no organ damage
Standard: Active monitoring. Trials testing early daratumumab or other intervention to prevent progression
Active myeloma in fit, younger patients
Standard: Induction (D-VRd or KRd), high-dose melphalan + autologous stem cell transplant, lenalidomide maintenance
Older or less fit patients where transplant is not appropriate
Standard: DRd (daratumumab, lenalidomide, dexamethasone) or D-VMP (daratumumab, bortezomib, melphalan, prednisolone)
Disease progression after one or more lines of therapy
Standard: CAR-T (ide-cel, cilta-cel), bispecifics (teclistamab), selinexor, CELMoD agents, pomalidomide-based combinations
BCMA-directed CAR-T cells (ide-cabtagene vicleucel and cilta-cel) are approved for relapsed myeloma. Next-generation trials test dual-targeting CARs (BCMA + GPRC5D), humanised constructs, and earlier-line CAR-T therapy.
Teclistamab (BCMAxCD3) is now NHS-commissioned. Newer bispecifics targeting GPRC5D (talquetamab) and FcRH5 (cevostamab) are in trials, along with combinations that improve T-cell persistence and reduce resistance.
Cereblon E3 ligase modulators (iberdomide, mezigdomide) are next-generation immunomodulatory drugs that are active even in lenalidomide- and pomalidomide-resistant myeloma. Combined with dexamethasone and daratumumab in trials.
Adding daratumumab to VRd (D-VRd) or KRd (D-KRd) as induction before transplant is being tested in large UK multicentre trials to achieve deeper responses and minimal residual disease (MRD) negativity.
